③ Preferred date and time * If possible, please include a range (e.g., starting time from ___ on ___ day)
④ Your desired plan and treatment time *Treatment time excluding the initial consultation and examination +20 minutes
(⑤ If you would like to change clothes, please write "I would like to change clothes." If you bring your own clothes, there is no need to write this.)
