■ Purpose and use of personal information collection ■
We will use personal information provided by customers (including medical history and physical condition) to the extent necessary for conducting our business. Specifically, we will use the information as follows:
- Name and contact information will be used for the purpose of confirming reservations, responding to inquiries, providing aftercare, and sending notices and announcements, etc.
・When information related to individuals, such as occupation, affiliated organizations, residential area, etc., is obtained, it may be used as statistical data for market analysis.
・Information on medical history (medical history), physical condition, lifestyle habits, living environment, etc. will be used for the purpose of research and analysis as reference information for deciding treatment areas, treatment methods, and advice for daily life, and as statistical data, and will be used within that scope. However, if the person agrees to the disclosure of information that can identify the individual, including part of the above information, as treatment examples, it may be disclosed to the extent of the person's consent.

The personal information acquired will not be provided to a third party without the individual's consent, except for the exceptions set forth in Article 23, Paragraph 1 of the Personal Information Protection Act.
