*We introduce routes that are easy to get to by car or on foot (bus), taking into consideration road width and slope.
1. Head north on Iwakura Nakadori Street, which extends from Iwakura Station, and turn east at Chogenji intersection (bus stop).
② Turn south (right) at the second intersection.
③It is the building that you will see on your left (east side).
To get to the parking lot, turn left (east) just before the building (north side).
*There is a parking lot on the north side of the building and stairs on the south side.

*Once you have parked your car or bicycle, please go around to the stairs on the south side of the building and come to the second floor.

② Location (Iwakura/Karasuma Oike)
③ Preferred date and time * If possible, please include a range (e.g., starting time from ___ on ___ day)
④ Your desired plan and treatment time *Treatment time excluding the initial consultation and examination +20 minutes
(⑤ If you would like to change clothes, please write "I would like to change clothes." If you bring your own clothes, there is no need to write this.)
(⑥ If you would like to receive "special individual adjustments", please state so.)
